Problem

Audit results are showing sys_id values or Created on dates as root records' display value.0

Solution

Reason for audit results showing sys_id or Created on date as record name is simply that particular table does not have display value defined. 

In order to "fix" the issue, you need to make sure that every table/class used as Root container has a proper display value field defined.
